John is 70 years old and is spending way too much time in hospital facilities, being known as a regular among the nursing staff. Utilizing the Role Modeling Assessment Tool, a proper evaluation of his condition and the potential dangers that threaten care strategies can be explored.

Stressors

His problems breathing are one major immediate stressor. He has issues breathing, and so has limited his physical activity severely. He admits to not doing much, which is a negative contributor to his body's ability to recover and repair years of damage being done. John also sounds like he is a little depressed. He admits that he wishes more people would care about him. This is a dangerous stressor because it could impact his body's natural ability to heal. When the mind is depressed, the body also suffers.

There are also a number of long-term stressors that threaten John's condition as well. John's vices are the major long-term stressors that impact his physical and mental well being. He has a long history of drinking and smoking, and his wife even divorced him because of his alcohol issues. These bad habits have done their damage on his health, but also serve to threaten future prevention strategies to stop his condition from getting worse.

2. Resource Potential

John does have amazing support from the nursing staff and the phyisicains who know him so well because of his many hospital visits. They know his health issues and situation, making it much easier to diagnose and treat issues that he is having. This is an excellent source of support, because half the work of diagnosis is done just the second he walks into the door, saving him precious time and ensuring better treatment. Although John does have a lot of support, he has little support elsewhere, which is concerning. He lives alone, with his closest relative 400 miles away. This means that it will prove difficult for hospital staff to ensure he is following up with recovery and prevention activities, because they do not have anyone to help facilitate his adaptation to it, which could be a dangerous situation for a man of his age.

John has some strengths, but also some unfortunate weaknesses. First, he is very relaxed and easy to teat and work with from a nurse's perspective. This makes diagnosis and involvement with the patient much easier and simplified. He is able to help support nursing and physician staff requests. Still, he has some serious weaknesses that also threaten potential care strategies. One of his major weaknesses is his affinity for smoking and drinking. He often smokes right after he is stabilized and nurses have reported that he comes in drunk often as well. This is a clear issue that is undoubtedly affecting his health and recovery, but John does not seem to want to change his behaviors and adapt to his new way of life that requires greater attention to his health and well being. He has been to the facility 16 times in a year and a half, showing a clear inability to adapt thus far which is concerning for future health care goals.
