The future reforms discussed in this article are associated with three major areas i.e. medical health records, health care information access, and organizational workflows. The prediction of the form and function of medical health records in 2030 is based on the fact that the current models of healthcare delivery are changing. While the transformation is mainly driven by information technology, health care organizations are either transforming themselves or being transformed by the marketplace. In light of the technological transformation, it's expected that the quality of patient care will improve in various ways by 2030 because of digital medical records. Therefore, the future of medical health records in 2030 will be characterized by the use of digital or electronic medical records. In light of health information access, the article discusses the likely impediments to this concept in 2030 and two recommendations to prevent these impediments that can be executed now. The final part of the paper identifies the most significant element that could embed into organizational workflows and its potential impact.

As models of healthcare delivery continue to change, health care organizations are either transforming themselves or being transformed by the marketplace. Since information technology is the most important factor contributing to the transformation, it has resulted in the development of medical technology. Medical technology is in turn changing the delivery of healthcare way with healthcare services are delivered, accessed, and managed. With the ongoing change from old record management to a data management system, the form of medical records in 2030 will be electronic or digital. The electronic/digital medical health records will be used to store patient information and faster and more accurate diagnoses through greater access of the information by physicians (Blumenthal, 2010). However, this transformation will be hindered by the limitation of information technology, which will be an impediment to health care information access in 2030. This impediment can be averted through incorporating certain aspects of EBM and considering EBM capabilities as part of wider mobile device implementation. The most significant health care bake in to embed in organizational workflows is alerts, which promote communication between physicians and improving evidence base for priority setting.
