¶ … Care Impact the Continuum of Care Plan

As the county assistant of Buncombe County Department of Human Services, I am in charge of Continuum of Care in our county I have created a plan from the Buncombe County, North Carolina Continuum of Care Program to help provide innovated and long-term health care services for families and individuals who are in need of a variety of programs and services. There are many people in Buncombe County who qualify through determining their needs including: mental and physical health status, age, disability, income, education, job status, dependents, veterans, single parents, domestic violence, rental assistance, and assisted living. The Buncombe County Department of Human Services has come together with other non-profit organizations to organize and determine the needs of the residents by looking at 2010 records. In meeting we have also used the information from the different programs: finances, target populations, processed applications, statistics of successful applicants, where more labor is being spent, landlord requirements and costs spent toward rental programs, mental health funding and treatment costs, target communities services, and comparing and contrasting the information to cover any other gaps that we found in the continuum care for Buncombe County (U.S. Department of, 2011).

To create a seamless continuum of care we determine the information we met and determined there are crucial programs and services that we must support to assist Buncombe County residents. Furthermore, the HUD and Section 8 housing is the largest most successful program in the county, and we do request that financial assistance to continue because these housing programs supply the largest support to the community to reduce homelessness. The committee supports to continue funding to the Buncombe County Hospitals' Outpatient Program for the elderly who need emergency in home health care for when they leave the hospital. This program is crucial in assisting health care providers within the hospital and doctors' offices in being able to return home with referrals, appointments for outside services meet with family, treatment plans with in-home and assisted living services. The Housing programs, HUD and Section 8 also support and communicate services and referrals to and from patients from the hospital to increase revenue by increasing the demand for housing to people who did not know they were eligible for help. Together these programs have other services for the Continuum of Care to continue through the support and services of Section 8 Housing, HUD and other Housing Programs, and the Assisted and In-House Living Programs can work together to ensure peoples' needs are met (U.S. Department of, 2011).
The Buncombe County can achieve a successful long-term care plan by organizing a future of reducing expenses, assisting the homeless, shorter application process and wait time for rental assistance, elderly healthcare, low-income self-sufficiency programs to encourage family members to enter the Right To Work Program and become more independent and look at their educational status.
