I came to Us from Trinidad and Tobago with big dreams, they did not happened right away. I had to work at many different capacities making a living. I wen through a lot since comming to the US in 1989. I also worked as a Nurses Aid with a disabled men then I decided to go back to school and I graduated with AAS Nursing from Kingsborough Community college in 1998. My GPA was not as I wanted it to be because I had to work full time and had a family to support.
The program I am applying for is RN to MSN with the concentration in Nursing Administration. I have been a Nurse since 1998. All of my Nursing Career I have been working in Beth Israel Medical center first as a Graduate Nurse and then RN/ Charge Nurse on the Medical-Surgical Nurse. Healthcare systems are stressed by limited resources and increasing demands on their services. Nurses, as the largest group of healthcare professionals, have experienced significant changes in their work life and environment as systems have tried to meet these challenges. As workloads become more substantial because of cutbacks and the number of nurses per patient diminishes, patients and healthcare workers are put increasingly at risk.
I would like the opportunity to make a statement to leadership regarding the Nurse patient ratio dilemma we are facing in our organization. According to the Massachusetts Nurses Association in a research study by federal Agency for Healthcare Research and Quality shows that patients do better with lower nurse to patient ratios. The research study found in given hospital unit the optimal workload for a nurse was four patients. Increasing the workload to 6 resulted in patients being 14% more likely to die within 30 days of admission. A workload of 8 patients versus 4 was associated with a 31% increase in mortality. I have not been able to find any studies related to the effects of the nurse to patient ratio in a dialysis clinic.
Reducing the nurse patient ratio would set a safety net for patients and nurses. It would provide a safer environment for patients because there would be more staffing available for each patient. Patient can feel more confident that their needs are being met by the medical staff and the nurse will have fewer distractions. According to International Council of Nurses, California has set mandatory upwardly adjustable minimum nurse patient ratios. An example of this shows staffing for a medial surgical ward on day shift would be four patients for each Register Nurse. According to the International Council of Nurses improvements reported since the implementation of the ratios include more than 3000 extra nurses employed in hospitals, decreased staff turnover and absenteeism, and public approval of the State government has increased. This would also be incentives for nurses to work in the field of dialysis. It would have an impact on patient outcomes and on nurse retention and recruitment. I feel, as a nurse, that working in a safe environment for patients and staff is of the upmost importance. Nurses have dialysis technicians to rely on, but they are only able to rely on them to maintain equipment and alert her if a change in the patient occurs. The majority of responsibility for patient care falls on the Register nurse.
My strongest allies in the fight to change the nurse to patient ratio include dialysis nurses, patients, and the dialysis technicians. The majority of nurses that work in dialysis feel strongly about this issue. The nurses feel overworked and stressed due to the nurse patient ratio. The care and safety of ten patients fall upon one Register Nurse which puts a great deal of pressure on them. The register nurse is responsible for pre and post treatment physical assessments of all patients, giving all medications, supervision of the technicians, operating kidney dialysis machines, initiating, monitoring, and discontinuing dialysis treatments, assisting physicians on rounds, calling in prescriptions, making doctors appointments for patients, assisting patients with transportation issues, patient education, and writing physician orders. Patients also feel the effects and some have voiced to me their concerns. They feel their lives are in jeopardy due to the nurse being over extended between patients. Dialysis technicians also experience the effects because it requires them to take on more responsibility for patient care. Many of the technicians have no formal medical training and have a limited knowledge of patient care.
Nurses, patients, and technicians can help elevate this problem by joining together in a united front to voice their concerns and demand a change be made. No one group alone can make a difference but together new policies can be implemented and enforced. Also contacting the Nurse Association and asking them for help in the situation would be a great benefit. According to their website the ANA advances the nursing profession by fostering high standards of nursing practice, promoting the rights of nurses in the workplace, projecting a positive and realistic view of nursing, and by lobbying the Congress and regulatory agencies on health care issues affecting nurses and the public. ANA is the voice of nursing. This voice is heard through policy development, lobbying, publications/newsletters, involvement and partnerships with other organizations, and its presence on the Internet. ANAs work addresses the needs of nurses in their professional settings as well as patient care. The ANA involvement in this national issue is critical.
Change is always difficult and resistance is to be expected. One of the barriers to changing the ratios is the continuing nursing shortage that we are experiencing. The Bureau
of Health Professions predicts that the current shortage of 150,000 nurses nationwide
will by 2020 grow to 800,000 nurses the area hospitals are continually recruiting for nurses. Most of the hospitals offer between $5000.00 to $10,000.00 sign on bonus for a one year commitment. Dialysis organizations would be the main opponent to the change due to the cost. Dialysis units are smaller and without the financial resources the major hospitals have. The increased cost would place a greater burden on the clinics which have a limited budget each year. It is estimated by the American Hospital Association the projected cost to change the nurse to patient ratio would be around $270 million dollars.
The organization would play a key role proposed changed. The dialysis organization would have to change their views toward nurse patient ratio. The company would also have to find money in the budget to allow for a change. This would cause an increase in cost to patients as well. I believe patients would agree to the increase if it would guarantee them higher quality of care. Nurses may also be willing to take a small decrease in pay to help facilitate the change. Dialysis nurses receive some of the best pay rates that I personally found. I make more money working in a dialysis clinic than in the hospital. In order for the change to occur I believe that all parties involve must be willing compromise. We are all part of the same system of healthcare.
The cost of such a change would be the main negative of such a proposed plan. A majority of the cost would be passed on to the patients insurance company. Many of the patients receive Medicaid or Medicare which means the increase cost would fall on the tax payers. My main point to counteract this position is that patient safety must be our first and foremost priority. Although we may spend more money to begin with for more nursing staff patient outcomes would offset the expense. Patients that have better outcomes from dialysis treatments spend less time in the hospital. When patients are in the hospital we do not get paid for their treatments. Hospital stays cst the company a loss of revenue.
Unfreezing the organizations ideas on nurse patient ratio will take a great deal of time and effort. It would take at least two to three years to socialize the idea with all the dialysis organizations and put together a proposal for the specific changes. It would take another year to initiate implementation. Major changes never occur overnight and must be thought out and planned thoroughly before implementing.

You will prepare a literature review on a current issue in gerontology. I recommend that you select an issue that you feel is important to older adults, and one that needs more attention than that already is addressed in this course (I chose "Nursing home quality of personal and facility in the USA" Your literature review assignment has several components:

1. An annotated bibliography including a minimum of 15 academic references (journal articles or books) plus 10 websites that address the same topic. For each source annotated, you are expected to provide a complete reference and a one paragraph summary of what can be learned from that source; I will e-mail you a example of the format of the review that is required.

2. A 250 - 500 word summary what can be learned by reviewing this literature including a comparison of what can be learned from all the sources taken collectively. This summary must consider the limitations of what can be learned from websites alone;

3. A 250- 500 word analysis of this literature in consideration of the extent to which gender, race, and social class contribute to controversy surrounding the issue you have studied;

4. You will use your first three products to prepare a short series of (5-10) open-ended questions. You will then check what you think you have learned by conducting a short interview with an older adult about the issue you have been studying. You are encouraged to submit your initial literature review to the instructor and a draft of your interview questions for feedback before proceeding to interview the older adult. You will then write a short summary of 200-500 words commenting on what if anything your interviewee taught you that was not previously revealed in the literature.

1 - U.S. Health Care Spending in an International Context, Uwe E. Reinhardt, Peter S. Hussey and Gerard F. Anderson, Health Affairs, 23 (3): 10?25, May/June 2004.

This study uses the most recent data from the Organization for Economic Cooperation and Development to explore why U.S. health care costs are so much greater than costs in other countries with much older populations. The authors point to several reasons for higher U.S. health costs: the fragmented financing system entails higher administrative costs; health care providers have greater market power than health care purchasers, allowing prices to soar above levels of other countries where the government exercises collective bargaining power; and the U.S. provides a more specialized, intensive form of care.


2 - Disease Management Programs in Germany's Statutory Health Insurance System, Reinhard Busse, Health Affairs, 23 (3): 56?57, May/June 2004.

This study focuses on the introduction of disease management programs in 2002 into the Germany's statutory health insurance pools, which cover about 88 percent of the population. An earlier reform had introduced consumer choice among the various "sick funds," resulting in adverse selection and disadvantaging the chronically ill. Disease management was introduced to help improve quality and cost-effectiveness of treatment for chronic conditions, specifically by setting evidence-based guidelines for treatment and drug formularies and by better coordinating care. The author notes that this approach may be of interest in the United States, where adverse selection has thus far hindered managed care efforts among the Medicare population.


3 - How Does the Quality of Care Compare in Five Countries?, Peter S. Hussey, Gerard F. Anderson, Robin Osborn et al., Health Affairs, 23 (3): 89?99, May/June 2004.

This article reports on efforts of the Commonwealth Fund International Working Group on Quality Indicators to compare the quality of care among different countries. The group used 21 indicators?such as five-year cancer survival rates, breast cancer screening rates, asthma mortality rates, and others?to compare the quality of care in Australia, Canada, New Zealand, the United Kingdom, and the United States. None of the countries consistently scored best or worst overall. For example, Australia scored highly on many of the indicators (cancer survival, breast cancer screening, asthma mortality) but had higher incidences of pertussis, or whooping cough, than other countries. In Canada, 30-day case fatality rates from acute myocardial infarction were higher than Australia and New Zealand in older age groups, but kidney and liver transplant survival rates were better than in the other countries. Rates of cancer survival were not as high as other countries in the U.K., while suicide rates were notably lower than other countries. By contrast, suicide rates in New Zealand?particularly among young people?were much higher than elsewhere. Breast cancer five-year survival rates and cervical cancer screening rates were highest in the U.S., and asthma mortality rates were increasing while they were decreasing in the other four countries.

1. The purpose of this 4-8 page paper is a formal concept analysis of one concept found in a nursing theory, adapting the methodology of Walker and Avant (2010, p. 157-179). The book is not needed to succeed with this assignment.

2. Any nursing theory may be used to find a concept for analysis. Only one concept is analyzed.

3. As part of the concept analysis, a limited and focused review of scholarly literature is conducted, using a minimum of 5 scholarly sources related to the selected concept. A formal literature review can be extensive as a writer endeavors to discuss and critique all the relevan litarature about a specific topic or area of knowledge. In this assignment, the level of effor is not required. What is expected is an integrated discussion of al least five articles on the concept selected for this assignment. In this integrated discussion, themes, ideas, or facts about the concept found in the reviewed fare presented in an organized fashion. Scholarly ciritisism of how the various aurthors arrived at those themes is uncluded. This critique relates the writers insights on such matters as the validity and reliability of trhe methods used by the different aurthors to learn about the concept and the strengths and weaknesses of those aurthors conclusions. Note: Research articles about the concept are the best kind of article for a literature review.

4. The steps in a concept analysis include: (1) selection of a concept used in a nursing theory (2) identification of the aims or purposes of the analysis (3) a review of selected scholarly literature on the concept (4) identification of possible use of the selected concept (5) determination of defining attributes (6) identification of model cases (7) identification of alternate cases (8) identification of antecedents and consequences. (9) defenition of empirical referents.

The paper concludes with a synthesis of the students knowledge about the concept. The scholarly literaure is incorporated throughout the analysis.

Criteria for Content
1. Introduction includes statements on the process of concept analysis, identifies the nursing concept and nursing theory from which it was obtained, and relates the main topics of the paper.
2. A substantive discussion of each of the following elements is present. All elements arwe supported by scholarly literature.

a. One aim or purpose of the analysis
b. A review of scholarly literature
c. two possible uses of the concept
d. three defining attributes
e. Two model cases
f. Two of the following alternative cases: borderline, related, contrary, invented, or illegitimate
g. One antecedent and one consequence
h. Two empirical referents

3. Concluding statements present the writers new knowledge on the concept acquired as a result of writing this paper.
