Description of the Assignment:
The purpose of this assignment is to synthesize one strategy for the application of a specific nursing theory to resolving a problem or issue of nursing practice in nursing leadership, nursing education, nursing informatics, or health policy.
Content
1. Introduction to the paper includes a few general statements on the idea of nursing theory being applied to solve problems/issues in nursing practice, regardless of the specialty area of practice. For example, why would one pick a nursing theory to solve a practice problem? Would a grand, middle-range, or practice theory be best? Does the writer have any experience in using nursing theory this way? In addition, a brief one-paragraph summary of a specific nursing theory and information on the sections of the paper are provided. The selected nursing theory can be a grand theory, a middle-range theory, or a practice theory.
2. Description of the problem/issue for which strategies will be developed. The problem to be resolved must be in nursing leadership, nursing education, nursing informatics, or health policy. Scholarly evidence supporting the issue is included. The problem/issue could be local to ones specific practice setting. For example, the setting might be a nursing unit, a nursing-education program, an informatics department, or a health-policy unit of a consulting firm. The problem/issue needs to be something that a nursing theory can impact, whether it solves the actual problem/issue or enables people affected by the problem/issue to deal with it. It is best if the problem/issue is from real life ??" something the writer of the paper has dealt with or is currently engaged in.

3. Applying concepts and principles from the selected theory, one strategy for resolving the identified issue is described in depth. Details on how the theory would be applied are included (consider: who, what, when, where, and how). Rationales for the strategy as well as evidence from scholarly literature are included. One ethical and/or legal aspect of the strategy is discussed. Expected outcome(s) from implementing the strategy are proposed.

The purpose of this assignment is to explore the empirical support and applicability of selected nursing theories to the nursing practice, education, administration and/or research. A clinical scenario will be provided to the students. Each group of students will select and describe ONE middle range theory and its theorist(s) appropriate to the scenario. Jill Watson has worked in a telecommunication manufacturing company for 7 years. She has organized numerous health fairs, led countless health help sessions, regularly posted health information on central bulletin boards, and provided screening programs for many illnesses. However, many still smoke, are overweight, do not exercise, and have other negative lifestyle habits.
Studies have shown that providing health information is a necessary but not a sufficient condition to promote positive behavioral or lifestyle changes. You will select a middle range theory to help Jill understand how to facilitate positive lifestyles among her clients.
Each group will prepare a paper (maximum of 15 pages long) that includes, but is not limited to the following areas:

The focus of this paper is on the application of a middle range theory to nursing practice. A brief description of the components of the theory should be presented in this paper. However, since this should have been done in your presentation, the focus in the paper will be on application. The proposed guidelines are based on Chinn and Kramer (2008).

Nursing theory is central to nursing practice. It is what helps to choose intervention. Kerlinger (1973) defined theory as a set of interrelated constructs, definitions, and propositions that present a systematic view of phenomena by specifying relations among variables, with the purpose of explaining and predicting the phenomena (as cited in Chism, 2010). Everything a nurse attempts to do is and or should be based on a grounded body of evidence that helps the nurse deliver the best possible care. That is where theory comes in. Theory can be used alongside decision making or integrated to teach decision making skills. The adult learning theory, reflective practice theory and problem based learning (Thompson & Stapley, 2011). Parse (1992) states that a nursing theory that assumes the existence of value for the patient regardless of any norms or expectations enables the nurse to practice in a holistic manner that does not attempt to change the patient or label him or her in any way (as cited in Cody, 2006). That is what nursing is all about, practicing holistically and doing what is best for the patient. That is what theory in nursing attempts to do.

The role of the DNP is to use the evidence that is gathered and constructed by theory which is in turn used to support their specific clinical practice (Zaccagnini & White, 2011). The DNP will essentially apply middle-range theories to practice, with the thought process being if one middle-ranged theory is learned and performed in practice, this will encourage the DNP to learn and acquire other mid-range theories (Magnan as cited by Chism, 2010). By doing this, it allows the DNP to practice with a greater knowledge base, and to effectively use theory in his or her practice. The AACN states that DNPs will use theories and concepts to detect and treat illness and improve health care delivery. They will also be able to describe the actions and advanced strategies to enhance, alleviate, and ameliorate health and health care delivery phenomena as appropriate; and evaluate outcomes (2006).

On a pulmonary unit the nurse must often teach patients with tracheostomies and ventilators how to communicate. Family members are often taught how to clean and care for tracheostomies when patients are discharged home with them. Many times patients are taught how to do nebulizer treatments and maintain oxygen therapy at home. Weakened patients are taught how to rebuild muscle strength in their arms and legs. Orems theory of self-care deficit is used numerous times throughout the day on this unit and has proven to be an asset to pulmonary nursing.

REQUIREMENTS:
Description of the Assignment
In this 5-6 page paper (not including the title or reference pages in the page count), the introduction contains a few statements about nursing theory in general, identification of
one nursing theory to be used in this paper, and the sections of the paper. The selected nursing theory can be from any of the three types of theories:
grand theory (e.g., Roy adaptation model),
middle-range theory (e.g., Benners model of skill acquisition),or
practice theory (e.g., Ims theory on the pain experience of cancer patients).
Note: the theories named here are examples only; students may choose any nursing theory.
1. The first section of the paper is a description of the importance of nursing theory, in general.
2. Key points and inter-relationships of those points of the selected theory are summarized. Biographical or historical information about the selected nursing theory or the nurse theorist is not included.
3. The selected nursing theorys views or ideas about nursing leadership, nursing education, health policy, or nursing informatics (only one of these specializations) are discussed briefly, using two examples from real life to illustrate the views. Real-life examples come from a students own practice or from the scholarly literature.
4. The paper is concluded by presentation of insights gained (what was learned) about nursing theory through writing the paper.
Criteria for Content
Introduction includes general comments on nursing theory (such as a definition of nursing theory and past exposure or experience with nursing theory), the name of the selected nursing theory, and the sections of the paper. A heading is not used.
Section one contains an analysis of the importance of nursing theory, in general, to the nursing profession. For example, one might address why nursing theory is studied and how nursing theory impacts the profession and patient care.
Section two is a summary of key concepts and relationships among the concepts of the selected nursing theory. Remember to state the name of the theory again. Avoid biographical and historical information about the theory or theorist. At a minimum, include reasons for selecting this theory, the purpose(s) of the theory, key concepts, relationships among key concepts, and how this theory addresses nursings metaparadigm. Students are not limited to these areas.
Section three presents a discussion of how the selected nursing theory views nursing leadership, nursing education, nursing informatics, or health policy, along with 2 real-life examples illustrating the views. If the selected theory does not explicitly discuss the chosen specialization, a possible view of the specialization can be proposed. Remember that real-life examples come from a students own practice experiences or from the scholarly literature. Keep each example succinct (about 1 ??" 3 paragraphs).
Section four is the conclusion. In the conclusion, ideas and understandings about nursing theory learned from writing the paper are shared.

Working in a Rural Health Family Practice clinic, diabetes mellitus has become a common diagnosis seen in most of the patients\' charts. With lack of exercise, increased unhealthy diet choices and overall, patient?s neglect to their own bodies, diagnosis new diabetes is an everyday norm for most providers and nurses. Using Kristen Swanson?s Theory of Caring, nurses need to be readily available to patients as they are diagnosed, learn to cope with new diagnoses, and manage their symptoms/diseases. Peterson & Bredow (2017) wrote, ?Swanson maintains that caring involves five processes: (1) knowing, (2) being with, (3) doing for, (4) enabling, and (5) maintaining belief? (pg. 140). As nurses, it is very important to remember that each patient is their own individual. No two patients are the same. As stated by the National Guideline Clearinghouse (2015), individualized care allows nurses to ?adopt an individualized approach to diabetes care that is tailored to the needs and circumstances of adults with type 2 diabetes, taking into account their personal preferences, comorbidities, risks from polypharmacy, and their ability to benefit from long?term interventions because of reduced life expectancy?. With an increase in HIPPA regulations, there is a corresponding increase of awareness regarding patient privacy issues among health care professionals as well as patients. This has led to the questioning of the need and appropriateness of IRB (Internal Review Board) approval for quality improvement (QI) initiatives. As Harnett (2009) points out the goal or purpose of QI is different than that of research, which requires approval by an IRB. In the realm of QI the study design is focused on improving the quality of care, whereas research involves a separate collection of data other than the findings currently in circulation (Harnett, 2009).
During this authors experience with a QI program on an oncology unit, the focus was to improve the management of blood sugars of patients who were on a sliding scale regimen. Data was collected from the electronic medical records (EMR) and was used in an educational seminar presented to the staff nurses. Evidence-based nursing related to management of blood sugar was introduced and the importance of managing patients blood sugars in order for healing to occur was emphasized.
If the QI initiative described above resulted in data that was then used as research evidence, there would have been a need for IRB approval (Newhouse, Pettit, Poe & Rocco, 2006). In this case the QI initiative did not progress any further that the initial educational presentation. However, there is an issue of human subject protection and the fact the EMR was accessed for this effort. It would be critical to ascertain who had access to the EMRs, and how was patient confidentiality and privacy protected, as this might be an area that would lead to the need of IRB approval (Lynn, 2004). As Newhouse et al. (2006) refers to this change in dynamics regarding QI and IRBs, it is a slippery slope that will continue to play out in the years to come.

The title of the article selected by this author is Theory-guided interventions for adaptation to heart failure. The objective of the study was to investigate the effects of a model-based experimental education, exercise, and social support program on adaptation in patients with congestive heart failure (CHF). The researchers used a randomized, parallel, controlled clinical trial involving 43 patients (21 intervention and 22 control patients). Inclusion criteria for participants included literacy, verbal communication ability, diagnosis of CHF for at least 6 months prior, ejection fraction of less than 40%, absence of visual or hearing defect, absence of mental disorder, absence of myocardial infarction within the past 12 month, and accessibility to telephone (Bakan & Akyol, 2007). Participants were issued with identification cards, given assessment forms for physiological data, the Minnesota Living with Heart Failure Questionnaire, interpersonal support evaluation, and were exposed to 6-minute walk test. The results obtained showed that patients in the intervention group adapted well to their condition, with enhanced quality of life, increased functional capacities and independence. Roys adaptation model was used by the researchers in the study.
Explanation, description, and prediction of nursing phenomena can be achieved using nursing models and theories (Mateo & Kirchhoff, 2009). Conceptual models cannot be tested directly hence the need to develop a middle-range theory, with the framework including both conceptual model and the middle-range theory. Roys adaptation model is a middle-range theory (Burns & Grove, 2009).
Adapting to a chronic illness such as congestive heart failure involves internal and external processes that affect responses and behaviors. The patient uses conscious awareness and choice to create integration with the goal of living with the realities imposed by illness (Whittemore & Roy, 2002). This study showed Roys Adaptation Model as an effective guide in nursing practice when caring for patients with CHF (Bakan & Akyol, 2007).
